Berserk!!! seems to have been a single-issue apazine published by Ken Cheslin, Fred Hunter and Archie Mercer for OMPA. It's listed as such in the British Fanzine Bibliography part three: 1961-1970 published by Peter Roberts, but in a mailing comment in Whatsit #8 (June 1964) Cheslin wrote:

I got a letter from. Fred Hunter in which he says he never did leave Shetland, never did go to London, or come here and help write Berserk!!!. I believe him. But. If it wasn't Fred Hunter...

Roberts spells the title as simply Berserk but Cheslin includes the triple exclamation mark when referencing the issue in Whatsit #8 as does Mercer in Amble #18 and as they were a co-editors they presumably knew what the title was supposed to be.
