Its publication seems to have been fraught. [[Norm Metcalf]] and [[J. Ben Stark]] teamed up with to produce it with Metcalf doing the research and Stark the publishing.  Metcalf seems to have been a perfectionist. Stark eventually gave up on waiting for him to finish and published what he had and Metcalf never forgave him.
