A comb-bound, 8.5x11", 34-page index to the prozine Unknown (later called Unknown Worlds) which was compiled by Stuart Hoffman, published in 1955, the only known publication of Sirius Press (Black Earth, WI. Motto: "At the sign of the howling Dog Star").

Introduction by Robert Bloch. It includes both author and title listings.

Not to be confused with Arthur Metzger’s 1976 An Index & Short History of Unknown that also contained author and title listings.
